Registering with us

We have an open list and welcome requests for registration from patients living in our practice area (5 mile radius)

Please come into the surgery to collect your registration pack.

When you register you will also be asked to fill in a medical questionnaire. This is because it can take a considerable amount of time for us to receive your medical records. All patients requesting to join the practice will be required to attend a new patient registration appointment with a member of our nursing team.

When registering, we will also need you to bring:

• Photographic proof of your ID, eg: passport or driving licence.
• A utility bill or bank statement with your name and current address (so we can check you live within our practice boundary).
• Details of any repeat medications you are being prescribed by your current GP.  A copy of your current repeat prescription request form or a verified printout from your current GP Practice will be required

Change of address

If you change your address please print and fill out and return our change of address form to the surgery.
